1. First step

Entry Form : Please complete Entry Form.

2. Second step

After submitting Entry Form, you will go to Wine Form page.
Please complete Wine Form.

If you are going to enter more than 10 wines, we suggest you to download the Excel form.

2-1 .Wine Form
Please complete Wine Form.

You are able to send your entry form and wine form using this specially designed Web form.

2-2 .Payment
Please select payment method and complete your payment following to instruction.
BANK TRANSFER
If you choose Bank transfer, you will access to bank details page.
For Japanese entrants※日本国内は銀行振込のみとなります。
CREDIT CARD
If you want to pay by credit card, please go to "Payment by Credit Card" page and print the Credit Card details form.
Please fill in the form and fax to 81 3 5825 2789.

3. Third step - Send your wines

- Send wines via courier to arrive by 3 July 2009 at our warehouse:

Japan Wine Challenge 2009,
Yokohama Warehouse, Shinagawa Futo Branch, Konan 5-4-12,
Minato-ku, Tokyo 108-0075 Japan. Tel. 81-3-5825-9177

- Each case of wine must be accompanied by a photocopy of the entry form and be clearly labeled as follows: your company name, Japan Wine Challenge 2009 and the number of bottles (e.g. 12) and case number (e.g.2/3).

- If you have a local distributor / agent, you may wish to arrange with them to deliver the wines ex-Japan stock (remember to complete the form stating you have done this).

- Please also mark on each case of wine "JAPAN WINE CHALLENGE 2009 - SAMPLES - NO COMMERCIAL VALUE".
